Understanding treatment

What are ED medicines, and how do they work?

Sildenafil and tadalafil belong to a group called phosphodiesterase type 5 inhibitors, or PDE5 inhibitors.

01

Support blood flow

They help maintain the body's blood-vessel relaxation signal during sexual stimulation, making it easier to achieve and maintain an erection.

02

Work with arousal

Sexual stimulation is still needed. A tablet does not create a continuous erection or automatically increase sexual desire.

03

Fit the person

The right choice depends on your health, medication interactions, preferred timing and experience with previous treatment.

Medication comparison

Sildenafil or tadalafil: what is the difference?

Both are established oral treatments. The main practical differences concern timing and how long the response window lasts.

What to compareSildenafilTadalafil
Recognizable brandsViagra, SidegraTalafil, Casey
Typical timingUsually taken around 1 hour before sex; response variesAs-needed treatment can begin working from around 30 minutes
Response windowA shorter window, usually several hoursCan last up to 36 hours
FoodA large meal can delay its effectFood has less effect on timing
Treatment patternUsually taken when neededAs-needed or prescribed daily treatment, depending on the plan
Choosing an optionYour physician checks health, interactions, response and preferences. A longer response window does not mean a continuous erection.

Use the schedule prescribed for you. Brand availability is confirmed with the clinic; do not combine ED tablets or change the dose yourself.

Who this may help

What can contribute to erectile dysfunction?

ED often has more than one contributor. Identifying the pattern helps the physician choose safer and more effective first-line care.

01

Blood flow and cardiovascular health

High blood pressure, diabetes, cholesterol, smoking and vascular disease can affect penile blood flow and may deserve broader medical attention.

02

Hormones, sleep and general health

Testosterone deficiency, thyroid concerns, poor sleep and other medical conditions may affect libido, erections or treatment response.

03

Medication and substance effects

Some prescription medicines, recreational substances, smoking and alcohol can contribute to erection difficulty or interact with ED treatment.

04

Stress and relationship context

Performance anxiety, mood, stress and relationship factors can affect erections, including in patients without an obvious physical cause.

Medication safety

How safe are erectile dysfunction medicines?

Sildenafil and tadalafil have an established role in ED care and are well tolerated by many patients. A brief health and medication review helps us select treatment with confidence.

  • Never combine PDE5 inhibitors with nitrate medicines for chest pain or recreational nitrites such as poppers.
  • Riociguat must not be combined with these medicines. Alpha-blockers and other medicines can also affect suitability or dosing.
  • Tell the doctor about heart symptoms, recent heart attack or stroke, low blood pressure, and kidney, liver or eye conditions.
  • If sexual activity has been medically discouraged, discuss that before starting treatment.

What to expect

Side effects and when to get help

Not everyone experiences side effects. Tell Kauri if a symptom bothers you so the treatment plan can be reviewed.

Common effects

Headache, facial flushing, indigestion, a blocked nose or dizziness can occur. Tadalafil can also cause back or muscle aches; sildenafil can cause temporary visual changes.

Get urgent medical care

An erection lasting 4 hours or longer, sudden loss of vision or hearing, chest pain, or a severe allergic reaction needs urgent assessment. Tell the treating team which ED medicine you took and when. Do not self-administer nitrates for chest pain after taking an ED medicine.

Getting a useful response

How to use ED treatment effectively

  • Follow the prescribed timing and instructions for your specific medicine.
  • Allow time for arousal and sexual stimulation.
  • Avoid heavy drinking, which can worsen erections and increase dizziness.
  • Tell your physician about all medicines, supplements and recreational substances.
  • Use medicines from a legitimate clinical or pharmacy source.
  • If results are disappointing, arrange a review before changing the dose.

One unsuccessful attempt does not settle whether a medicine can help. The next step is to review how it was used and whether another factor needs attention.

Your visit

What happens during an ED consultation at Kauri?

The visit is direct, private and clinically focused. You do not need to know the cause or choose a medicine before arriving.

  1. 01

    Describe the concern

Discuss when the problem began, how often it occurs, erection quality, morning erections, libido, ejaculation and previous treatment.

  2. 02

    Review health and medication safety

The physician reviews cardiovascular and metabolic health, current medicines, supplements, alcohol or substance use and whether sexual activity is medically safe.

  3. 03

    Examine or test when relevant

A focused examination or blood tests may be recommended to investigate glucose, cholesterol, testosterone, thyroid function or another suspected contributor.

  4. 04

    Create and review the plan

Discuss appropriate oral medication, correct use, underlying health factors, follow-up and referral when more specialized treatment is needed.

Ongoing care

Can ED medicines be used long term?

Some patients use ED medication over an extended period, with review of effectiveness, side effects and changes in health or other medicines.

Continued treatment is an individual decision. We also address contributors such as diabetes, blood pressure, smoking, sleep, stress and hormone concerns where relevant. Medication can support erections while these factors are being managed.

At follow-up, tell us what improved, what did not and whether the timing fits your life. That information helps guide the next prescription or referral.

Connected specialist care

What happens if tablets are unsuitable or do not work?

Not responding immediately does not always mean oral medication has failed. Kauri first reviews correct use, timing, tolerability, treatment expectations and whether an underlying contributor needs attention.

When first-line medical treatment is unsuitable or remains ineffective, we can recommend evaluation by a urologist or another appropriate specialist.

  • Penile injections or intraurethral treatment
  • Vacuum erection devices
  • Assessment for selected procedural treatments
  • Penile implant consultation
  • Complex neurological, vascular or post-surgical ED
  • Psychosexual or relationship-focused therapy

Patient questions

ED medication and treatment: frequently asked questions

Can Kauri treat erectile dysfunction?

Yes. Kauri provides private physician assessment and oral ED treatment with sildenafil or tadalafil when appropriate, with testing and follow-up tailored to your health. Specialist referral is available when another treatment is needed.

Do ED tablets increase sexual desire?

No. Sildenafil and tadalafil support the blood-flow response to sexual stimulation; they do not directly increase libido. Reduced desire deserves a separate review of health, hormones, medicines and emotional factors.

Is tadalafil better than sildenafil?

Neither is best for everyone. Tadalafil offers a longer response window, while sildenafil suits many people who prefer a shorter planned window. Your physician considers response, side effects, medicines and preferences.

Are Sidegra and Viagra the same medicine?

Both contain sildenafil. The brand, strength and availability can differ. Kauri reviews the active ingredient and appropriate prescription rather than choosing by brand alone.

Are ED medicines safe?

For many patients, prescribed ED medicines are effective and well tolerated. Safety depends on your cardiovascular health, other medicines and correct use. Never combine them with nitrates or poppers.

Can I take ED medicine with my usual medication?

Your physician must check the combination first. Nitrates and riociguat must not be combined with PDE5 inhibitors; alpha-blockers and some other medicines also require review. Bring a full medication and supplement list.

Will I need blood tests?

Tests are selected according to your symptoms and health history. Glucose or HbA1c, cholesterol and testosterone may help identify contributors; additional tests are arranged when relevant.

Does low testosterone always cause ED?

No. ED has several possible causes. Low testosterone can affect desire and erections in some patients, but TRT is considered only when evaluation supports testosterone deficiency.

What if the first tablet does not work?

Arrange a review before increasing the dose or mixing treatments. Timing, food, stimulation, anxiety and underlying health can affect response. Your physician can adjust the plan or recommend specialist care.

Do I have to take ED medication forever?

No. Treatment duration depends on the cause and your response. Some people use medication when needed; others benefit from ongoing treatment and periodic review. Treating contributing health problems can change what you need.
