Work Permit Medical Certificate in Bangkok

Understand the standard Thai work-permit health declaration, the examination and the results needed—then plan a straightforward visit at Kauri.

Your question, answered

What is a work-permit medical certificate in Thailand?

It is a doctor-signed health declaration used for a Thai work-permit application. The physician assesses the six prohibited conditions under the relevant regulation and completes the required medical findings, including the syphilis blood-test result. Kauri’s pathway includes medical history, vital signs, physical examination, syphilis testing and drug screening.

Who needs a work-permit health assessment?

Foreign nationals whose work-permit application checklist requires a medical certificate need the relevant physician assessment. This can apply to a new application or a renewal. A work permit, a visa and an employer’s internal health record are different documents; completing the medical visit does not itself grant permission to work or extend a visa.

Standard applications and special migrant-worker registration programmes do not always use the same health-check pathway. Some programmes specify participating or approved hospitals. If your employer is using such a programme, confirm its facility requirements first. For a standard certificate, you do not need an employer-created form to begin the consultation.

Which six conditions are assessed?

The regulatory list addresses leprosy, infectious tuberculosis, drug addiction, chronic alcoholism, elephantiasis and tertiary syphilis. Some entries specify a particular stage or severity. The doctor also considers the mental-health wording required by the official declaration. The assessment concerns the defined criteria—not a blanket exclusion of everyone with a past illness or mental-health diagnosis.

Which six conditions are assessed?
LeprosyInfectious stage or manifestations specified by the regulation
TuberculosisInfectious tuberculosis
ElephantiasisThe symptomatic stage specified in the regulation
Drug dependenceRelevant substance history and clinical findings
Chronic alcoholismChronic alcohol dependence, not simply any alcohol use
Tertiary syphilisA defined clinical stage; a screening result alone does not establish it

What does the examination at Kauri include?

Your visit includes a review of medical history and current medicines, vital signs such as blood pressure and pulse, a physical examination, a syphilis blood test, drug screening, and the physician’s assessment and signed certificate. The doctor reviews the findings before completing the declaration.

Drug screening is part of Kauri’s stated pathway. The government certificate defines the health conditions to assess; it should not be read as a universal instruction that every applicant in every permit category needs an identical laboratory package. Additional job-specific tests can be considered if an employer requests them.

Why is a syphilis blood test included?

Syphilis testing supports the assessment because the regulated health declaration includes tertiary syphilis. Kauri includes the blood test in this pathway. A reactive screening result does not by itself establish tertiary syphilis. Previous treatment and different types of antibody tests can affect interpretation, so bring old results or treatment records if relevant. The physician may need clarification or follow-up before completing the certificate.

How recent must the medical certificate be?

Use the medical-certificate age limit stated in your current Department of Employment application checklist. The medical document’s submission period is separate from the duration of the work permit itself. Confirm the submission date before arranging the examination; a one-year work permit does not make its supporting medical certificate reusable for a year.

For example, the official WP.25 renewal checklist hosted by Mahidol University specifies the certificate period stated by the physician. This is why a blanket “30 days” or “1 year” answer is not accurate for every route. Ask for the relevant period to be clearly recorded, and follow the current checklist for your own application.

How do I prepare for the work-permit medical visit?

Bring your passport, your existing work permit if renewing, a medication list and any relevant medical records. No fasting is normally needed for the standard visit. Tell the team about previous syphilis treatment, long-term illness or recent medications that may matter for result interpretation. Do not stop prescribed medicines on your own.

If you have old X-ray films or reports, please bring them because they may help the doctor make an assessment. For additional employer-requested tests, follow the preparation instructions for those tests. The usual Kauri visit is 30–45 minutes, with same-day completion when the assessment and required results are available.

Kauri’s pathway includes medical history, vital signs, physical examination, syphilis testing and drug screening.

Can I use this assessment for a work-permit renewal?

Yes, where your renewal checklist requires this medical certificate. Bring your current permit and check the submission window for the renewal route you are using.

Is a positive syphilis test automatically a failed certificate?

No single screening result should be interpreted alone. The doctor considers the test type, clinical findings and previous treatment. Tertiary syphilis is a specific clinical stage, not another name for any positive antibody result.

Do I need to fast for the work-permit blood test?

Fasting is not normally required for the standard syphilis test and examination. If you arrange other tests at the same visit, follow the instructions for those additional tests.

Can Kauri assess several employees together?

Contact the team with the number of people and preferred date so appointments and sample collection can be coordinated. Each person still needs an individual physician assessment and accurate identification.

What can delay the completed certificate?

Missing identification, a result needing confirmation, extra tests or a programme-specific requirement can add time. Bring relevant records and arrange the visit with enough time before your application deadline.
