Five-Disease Medical Certificate in Bangkok

A clear explanation of the familiar “5-disease certificate”: what the doctor assesses, where the document is used and why the correct format matters.

Your question, answered

What is a five-disease medical certificate in Thailand?

A five-disease medical certificate is a commonly requested health document for employment, education or administrative applications. Traditionally it addresses leprosy, tuberculosis, elephantiasis, drug dependence and chronic alcoholism. The doctor reviews your health and signs the declaration appropriate to its purpose; it is not automatically a package of five laboratory tests.

When is a five-disease certificate used?

Employers may request a general health certificate before you start a role. Schools, universities, training programmes and licensing bodies may also request health documentation. Tell the clinic the purpose and the submission date; the assessment can then address the relevant health declaration rather than an unrelated screening package.

The name is widely used, but forms have evolved. The Medical Council’s general health certificate and a specific organisation’s “five-disease” form do not necessarily contain identical wording. If you already have an application form, bring it. If not, start by telling Kauri where the certificate will be submitted.

Which five conditions are commonly named?

The traditional list is: leprosy at an infectious or specified symptomatic stage; tuberculosis at the stage stated on the form; elephantiasis with specified visible manifestations; drug dependence; and chronic alcoholism. The wording concerns particular clinical findings or stages, not a blanket judgement about anyone with a past diagnosis.

The examination combines your history and current findings. A brief examination cannot prove that every infection is absent. Where symptoms, past treatment or the requested declaration need more evidence, the doctor selects relevant investigations and explains the next step.

Is this the same certificate used for civil-service recruitment?

Not necessarily. The OCSC disease rules were updated in B.E. 2566 (2023) and list four categories: elephantiasis at the specified stage, drug dependence, chronic alcoholism, and serious communicable or chronic disease with pronounced or severe symptoms that obstruct duties. Do not assume that an older five-disease list satisfies a current civil-service vacancy.

Recruitment notices can also specify a government hospital, particular examinations or a recent issue date. Check that part of the notice before booking. This is different from the general private-employer health certificate offered at a clinic.

What happens during the five-disease assessment?

The physician asks about current symptoms, previous conditions, medicines, treatment history and relevant substance use, then performs a focused physical examination. Measurements such as pulse and blood pressure help document your health at the visit. Additional blood, urine or other investigations are based on the purpose and clinical need—not simply the number “five”.

How long does the certificate take, and how long is it valid?

A straightforward assessment can be completed during the visit when no additional results are needed; Kauri confirms your expected timing when arranging the appointment. There is no universal validity period for every document called a five-disease certificate. The employer, school or authority sets how recent the examination must be. Arrange it close to the submission date.

What should I bring for a five-disease certificate?

Bring your Thai ID or passport, the purpose of the certificate, medication details and relevant previous results. Bring any requested form if available. If you have old X-ray films or reports, bring them because they may help the doctor’s assessment. Fasting is not usually needed for the examination alone; follow any instructions for separately arranged tests.

Traditionally it addresses leprosy, tuberculosis, elephantiasis, drug dependence and chronic alcoholism. The doctor reviews your health and signs the declaration appropriate to its purpose; it is not automatically a package of five laboratory tests.

Does “five diseases” mean five blood tests?

No. It describes a health declaration, not a fixed laboratory panel. The doctor uses your history and examination and arranges relevant testing when needed.

Can this replace a work-permit medical certificate?

Use the work-permit assessment for a work-permit application. That declaration addresses six specified conditions, including tertiary syphilis. Kauri’s work-permit pathway includes a syphilis blood test and drug screening.

Does the certificate prove I have no health problems?

It records the findings relevant to the stated purpose at the time of examination. It is not a comprehensive health check or a guarantee that every illness is absent.

Can I attend without an employer’s form?

Yes. Tell Kauri which certificate you need and where it will be submitted. A supplied form is useful only when the receiving organisation requests particular wording or checks.

What if I have a previous diagnosis or ongoing treatment?

Bring the relevant records and discuss the current situation with the physician. The assessment considers present findings and the specific declaration; a past diagnosis is not the same as a current disqualifying condition.
